A referee could start the campaign in Elturel, and the players are in the city when it is sucked down to hell - living through the apocalypse and scrounging to survive instead of arriving for the aftermath. My group started playing the campaign and there was zero enthusiasm from any of us -- including the GM! -- once it became clear how linear the path was to the advertised "good bit" of the adventure.
